I have seen many requests like that when surfing forums of all types regardless if they are business forums or shady underground forums all the time the same question. How can I urgent get a offshore company 100% anonymous. Fact is, you don't get it anywhere, there is no such service any longer.Say for 10 or 15 years ago you may have been able to find services like that but today you will need to provide some sort of documents like passport copy and utility bill as an minimum. If you need offshore banking for your company you will even need to provide notarized documents (depending on the bank) and bankers reference letters may some banks require as well.

Yeah it's going to be difficult to find any service that offer anonymous offshore company formation! What some may try is to appoint nominees i.e. shareholder and director when they form the company that may help to increase privacy dramatically.

For urgent company formation offshore you may look at jurisdictions like Belize, Seychelles, BVI, Panama, and Isle of Man they can be registered within a few days.

It's almost correct, nominees provide you with an additional laywer of privacy and anonymity, however, not all countries have this option and allow nominees.
 
Nominee director and shareholder are not doing it alone sometimes you wan to put more into it I think. Just asked same question in other thread to hope to get answers.
