The Atlantico Bank in Portugal: I think it's around 9EUR/month. You'll have an EUR account but can ask for a USD account as well.

DukasCopy (CHF and USD) in Switzerland: free if you hold 40 "dukas coins", which is a crypto token they mint, that's around 50~60USD. No maintenance as long as you keep those coins (may have changed, it's been a while since I used the account).

They are quite strict with the KYC process and require live video calls to see your passport and sign the papers. So, it may take a few days because they operate in Swiss time zone and you need to be available during that time for the call to happen.

Other than that, I have the suspicion that they don’t want to deal with US citizens, as multiple times they have asked if I was a US person.

What made me stop using the account is that they assign a maximum quarterly value that you can receive in the account that is based on the proof of income you provided when opening the account (eg: you earn $1.000 then they assign you a quarterly maximum of, say, 3.000$ plus something). You can request an increase, but at the time I was planning to increase my income by 3x and I didn’t have a way to prove it in advance. So I had to switch to another bank.
